最长肌是在半棘肌侧面的肌肉，为竖脊肌最长的一段，向上伸展至颈椎后路的横突上。

背最长肌是竖脊肌中居于中间且最大的一段。

在腰部，背最长肌和腰髂肋肌相混杂。部份纤维附在腰椎横突和副突的整段后表面上，以及胸腰筋膜]的前层上。

在胸部，背最长肌以圆腱附着至所有胸椎横突的唇上，且以肉突附着至下方九或十根肋骨的结节和角之间。
